Copy and paste the questions into a word document. Answer the questions and then print the page. Write your name on the back and hand this in.

1. When is your birthday?
2. Do you have any siblings? If so, how many?
3. Do you have any pets? If so, tell us about them.
4. List five words to describe you (adjectives).
5. What are you most proud of?
6. What do you enjoy doing in your free time?
7. What are your hobbies?
8. Who is your favorite singer/athlete?
9. If you could bring just one thing with you on a long trip what would it be?
10. What would you like to be when you are older? (Besides an athlete)
